Name of the Birthday Person
Name or Title on the shirt ( mom, dad, sister...)
Age they are turning 
 
You can order the number of shirts you want and let me know below the names and sizes for each shirt or you can add each shirt to your cart separately. 
 
A design will be create and sent to you for approval before the shirts are made.
